Community Health Assessment
What is a Community Health Assessment (CHA)?
A process by which community members gain an understanding of the health concerns and health care systems in our community. This is accomplished by identifying, collecting, analyzing, and disseminating information about community assets, strengths, resources, needs and provides guidance to address health care needs and fulfill both federal and state mandates.
In order to assess the overall health needs of Trego County residents, the Trego County Health Department in collaboration with the Trego County-Lemke Memorial Hospital (TCLMH) has completed a Community Health Assessment and Community Health Improvement Plan.
